Reducing Hospital Readmissions

Many patients discharged from a hospital return within 30 days for the same or related health conditions. This can rack up unnecessary expenses and cause further health issues ... both of which are preventable. Favor Home Care can work with you to help reduce the risk of readmission by providing high-level, in-home care immediately following discharge.

We understand patients often have trouble performing the regular activities of daily living following a hospital discharge. Injuries and ailments may make it difficult just to get out of bed and take care of personal hygiene and nutritional needs. We not only help you with these needs and other home care tasks following discharge, we also help make accommodations so you will be able to perform these activities independently as soon as possible.
